Our Commitment to Your Privacy
At Sonotec, we understand that your medical information is personal and we are committed to protecting it. We create a record of the care and services you receive to provide quality care and to comply with legal requirements. This notice applies to all of the records of your care generated by our medical imaging facility.
How We Use and Disclose PHI
We may use and disclose your Protected Health Information (PHI) for treatment, payment, and healthcare operations. For example, we may share your ultrasound results with your referring physician to coordinate your care, or provide information to your insurance provider to receive payment for services rendered.
Your Patient Rights
- As a patient at Sonotec, you have specific rights regarding your protected health information (PHI). You have the right to inspect and copy your medical and billing records, request an amendment to your PHI if you believe it is incorrect, and receive an accounting of certain disclosures we have made of your information.
- You may also request that we communicate with you in a specific way or at a certain location to maintain your privacy. To exercise any of these rights, please submit a written request to our Privacy Officer at hello@sonotec.com.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with Sonotec or with the Secretary of the U.S. Department of Health and Human Services. All complaints must be submitted in writing. We will not retaliate against you for filing a complaint.
